Commander: Carey Slade was a Builder 2nd Class (Sea Bee’s) when she was honorably but medically discharged. She proudly served for 4 years and 11 months. She completed her Builder “A” School training in Gulf Port MS graduating with honors and 3rd in her class. She went on to be stationed in NAF Atsugi, Japan Public Works for three years, were she received letters of appreciation and awards for a job;s well done . While stationed at NAF Atsugi; she attended 40hr training for the Honey Well computer system being installed at transportation and assisted in computerizing the base fleet of vehicles maintenance schedule along with her regular duties as a Builder. Her next duty station was Point Loma Submarine Base, Public Works in San Diego CA. where she volunteered for the Auxiliary Security Force attending 40 hours of training monthly, also was a volunteer for the V.I.T.A. program assisting military personnel with the taxes, along with her regular duties in the Public Works department. While serving she was awarded her Good Conduct Medal and 3 Sea Service ribbons.
After her honorable but medical discharge; she went on to work at BofA Credit Card Fraud Unit where she excelled at stopping fraud at the point of sale. She also has been writing and playing music for over 30 years and has released 3 solo CD’s and 3 full band CD’s.
Carey has always excelled at what ever is put in front of her; with her “Can Do” attitude and attention to detail, she is well suited for the position of Commander.

Adjutant -Dr. Carl Forkner received his Doctor of Philosophy in Higher Education Administration in November 2013. He previously received a Bachelor of Arts Degree in History in 1980, a Master of Science in Education in 1984, and a Master’s Degree in Strategic Studies and International Security Affairs in May 2006.
After three years teaching at high school and undergraduate collegiate levels, Dr. Forkner served as a commissioned officer in the United Stated Navy from 1983 to 2011. He was designated a Naval Flight Officer and later a Surface Warfare Officer, flying multiple tactical aircraft and operating at sea on 2 submarines, a cruiser, a destroyer, and 12 aircraft carriers.
Operational experience during his Navy career included eight overseas deployments. He served in three counter-terrorism operations, including the 1986 Operation El Dorado against Libya, 2001 Operation Enduring Freedom off Afghanistan, and Operation Noble Eagle & Presidential Support while attached to NORAD from 2002-2005. In addition to DAV, he is also a life member of Veterans of Foreign Wars (VFW), Military Officers Association of America (MOAA), the American Legion, and the Navy League.
In addition to serving as a full-time DAV Volunteer, Dr. Forkner volunteers assisting career-seekers at Career Connectors (a Phoenix area nonprofit), is a certified Dementia Friends AZ Champion and trainer, and a Local Advocate & Trainer for the American Foundation for Suicide Prevention.
